Job Type: Part-time; Schedule: Approximately 18–22 hours per week

Harmonia Collaborative Care is looking for a compassionate, organized Social Care Navigator to help community members connect with services and resources that support their health and well-being.

• Complete social care screenings
• Connect individuals with community resources and services
• Make referrals and conduct follow-up
• Verify eligibility and maintain accurate documentation
• Communicate with clients, providers and community partners
• Help people navigate barriers to needed support

What We’re Looking For

• High school diploma or equivalent
• 1–2 years of experience in care coordination, healthcare, community services, public health, nonprofit services or a related field
• Strong communication and organizational skills
• Comfortable working with individuals from a variety of backgrounds
• Community Health Worker certification is welcome but not required if other qualifications are met
