What You’ll Do

Coordinate care for individuals enrolled in the Health Homes program

Develop and monitor individualized care plans

Connect members to medical, behavioral health, and social service resources

Maintain accurate documentation and compliance with program requirements

Collaborate with providers, families, and community partners

What We’re Looking For

Bachelor’s degree in a human services field or equivalent experience

Strong organizational and communication skills

Ability to work independently in the community

Commitment to person-centered, strengths-based care
