Case Manager – Health Homes

We are seeking a Case Manager – Health Homes who is organized, compassionate, and committed to doing work the right way.

What You’ll Do:

• Coordinate care for individuals enrolled in the Health Homes program
• Develop and monitor person-centered care plans
• Build strong, respectful relationships with members, providers, and community partners
• Connect individuals to medical, behavioral health, and social service supports
• Maintain accurate, timely documentation aligned with program standards
• Contribute to a team culture grounded in accountability, collaboration, and excellence

What We’re Looking For

• Bachelor’s degree in a human services field or equivalent experience
• Strong organization, communication, and follow-through
• Ability to work independently in the community and collaboratively within a team
• Commitment to strengths-based, person-centered care
