What you’ll do: Coordinate care for Health Homes membersMaintain person-centered care plansCollaborate with providers and community resourcesConnect members to medical, behavioral, and social supportsComplete timely Medicaid documentation.

What we’re looking for: Bachelor’s degree or related experienceCare management or community-based experience preferredStrong organization, communication, and follow-through.
